Question: Can I use an Alcohol testing form that is a single piece of paper instead of the carbonless 3 part form for a DOT test; and make copies of the original (for pages 2 & 3 from it using a standard copier) if I attach an original printout showing test number etc. to each?

Answer: Section 40.225 (a) through (b)(6) in the 49 CFR Part 40 states that "the DOT Alcohol Testing Form (ATF) must be used for every DOT alcohol test begining February 1, 2002. The ATF must be a three-part carbonless manifold form...You may view this form on the ODAPC web site (http://www.dot.gov/ost/dapc)."
